You do not need a long checklist this October. You need the three moves that matter most.
Dr. Okafor is a hospitalist who picks up locum weekends. Her hospital paycheck has taxes withheld. Her 1099 locum income does not. She also has an old rollover IRA from residency and plans a backdoor Roth in December. Each piece seems fine on its own. Together, they create three quick wins she should claim now.
Locum tenens, moonlighting, and consulting income is often paid with nothing withheld. If your prior year adjusted gross income was above $150,000, paying at least 110% of last year's total tax is generally the safe harbor for avoiding an underpayment penalty.
Your move: Make an estimated payment by January 15, 2027, or ask your employer to increase your W2 withholding for the rest of the year. Withholding is generally treated as paid evenly through the year.
The backdoor Roth can be a smart move for high earning physicians. But the pro rata rule looks at your traditional, SEP, and SIMPLE IRA balances on December 31. A pretax balance in an old rollover IRA may make a large part of your conversion taxable.
Your move: If your employer plan accepts incoming rollovers, consider moving that balance before year end.
Contributions to your 403(b), 401(k), or 457(b) may reduce this year's taxable income. If you have 1099 income, a Solo 401(k) may let you save more. Your employee deferral limit is shared across plans, but employer contributions from self employment income may be available separately.
Your move: Check your totals against the 2026 limits and adjust your remaining paychecks.
Each win rests on documentation, intent, timing, and structure. In October, timing is your advantage. After December 31, these options close.
Depending on your circumstances, one of these may matter far more than the others. A proactive review can help you find out which one while there is still time to act.
